At Connected Kids Chiropractic, we are committed to protecting the privacy and confidentiality of your health information. We take reasonable steps to protect your health information from unauthorized access, use, or disclosure. This includes electronic safeguards, locked storage areas, and staff training.

1. Acceptance of Terms
By scheduling an appointment and receiving services at Connected Kids Chiropractic, you (as the patient and/or parent or legal guardian of the pediatric patient) agree to be bound by these Terms and Conditions. These terms govern the use of our services, including chiropractic evaluations, treatments, and any related care.
2. Nature of Services
Our clinic provides chiropractic care specifically for families and pediatric patients. Services may include but are not limited to: spinal adjustments, physical examinations, developmental assessments, posture evaluations, and wellness consultations. All treatments are provided by licensed chiropractic professionals in accordance with the laws and regulations of the State of Tennessee.
3. Medical Disclaimer
Chiropractic care is not a substitute for medical or emergency care. If your child is experiencing a medical emergency, you should call 911 or seek immediate care from a hospital or medical doctor. Our services are complementary to traditional pediatric healthcare and are provided for musculoskeletal and related concerns.
4. Parental Consent
As the legal guardian, you must consent to treatment before any services are provided to a minor. You may be asked to sign a written Informed Consent form outlining the nature of the care, potential benefits, and any associated risks.
5. Appointments and Cancellations
We request at least 24 hours' notice for any cancellations or rescheduling.
Missed appointments or late cancellations may be subject to a cancellation fee.
Repeated no-shows may result in discharge from care at the provider’s discretion.
6. Payment
Payment is due at the time of service unless other arrangements have been made.
We accept cash, credit/debit cards, and HSA/FSA plans.
7. Privacy and HIPAA Compliance
We are committed to protecting your and your child’s privacy in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and Tennessee state law. Please refer to our Notice of Privacy Practices for more information on how health information may be used and disclosed.
8. Minors and Supervision
Patients under the age of 18 must be accompanied by a parent or legal guardian for all visits unless prior written consent has been given for another adult to accompany the child. We reserve the right to refuse treatment if proper supervision or consent is not provided.
9. Termination of Care
We reserve the right to terminate the provider-patient relationship at any time, including but not limited to: non-compliance, repeated missed appointments, or inappropriate behavior. A written notice will be provided in accordance with Tennessee Board of Chiropractic Examiners regulations.
10. Limitation of Liability
By accepting care, you acknowledge that chiropractic care, like all health care, has risks. Our providers follow accepted chiropractic procedures and safety guidelines. However, we are not liable for any outcomes that arise from misinformation provided by the patient or guardian, or misuse of the services provided.
11. Changes to Terms
We reserve the right to modify these Terms and Conditions at any time. Updated versions will be made available on our website and/or in our office.
12. Governing Law
These Terms and Conditions are governed by the laws of the State of Tennessee. Any disputes arising out of the use of our services shall be resolved in accordance with Tennessee law.
